Our caregivers assist with bathing, grooming, dressing, toileting, transfers, ambulation, meal preparation, and medication reminders. Beyond assistance with activities of daily living, we observe for meaningful changes in condition, including increased shortness of breath, edema, confusion, weakness, appetite decline, medication intolerance, or increased fall frequency.
Care is not delivered in isolation. Nursing leadership establishes the care framework, defines risk areas, and remains available for escalation when symptoms shift. If a client begins showing signs of decline, we intervene early by communicating with family and providers before the situation becomes emergent.
This model is particularly appropriate for individuals managing diabetes, cardiac disease, early to mid-stage dementia, Parkinson’s disease, or those transitioning home after hospitalization who still require structured monitoring.
