A safe discharge does not end at the hospital door. Liberty Home Care is structured to extend the discharge plan into the home environment with RN-directed oversight, clear communication, and defined escalation pathways. We reinforce discharge instructions, support medication adherence, monitor changes in condition, and provide structured supervision designed to protect stability during the critical transition period. Our goal is to ensure that the plan created in the hospital is carried out consistently and safely at home.

At the same time, we understand the importance of supporting hospital readmission reduction efforts. While no provider can eliminate readmissions entirely, our clinical model is designed to identify risks early, respond promptly to changes, and reinforce patient and family education before concerns escalate. Through proactive oversight, structured care planning, and timely communication, we work to reduce avoidable returns and support sustained recovery in the home setting.

Dr. Keisha Rollins-Munroe, PhD, MSN Ed., BSN, LPN — Director of Nursing
Dr. Rollins-Munroe brings more than 21 years of nursing experience across intermediate care units, emergency services, population and community health, nursing management, and nursing education. Beginning her career as an LPN in 2004 and advancing through BSN, MSN in Nursing Education, and a PhD in Nursing Education and Administration, she leads clinical oversight, care planning standards, supervision protocols, and quality assurance across all cases.

Robin Peace, BSN, RN — Executive Director of Clinical Operations
Robin is an experienced healthcare executive with a demonstrated history of working in hospital and home care environments. She holds a Bachelor of Science in Nursing and brings extensive experience in critical care nursing, patient advocacy, complex case coordination, and home care operations. She has also led and owned assisted living facilities, providing a deep understanding of post-acute transitions, regulatory expectations, and continuity across care settings. In her role, Robin oversees clinical operations, reinforces nursing leadership, and ensures structured execution of care plans across active cases.
