Clinical Operations Manager
(Registered Nurse)

About the Role:
Liberty Home Care is seeking a compassionate and experienced Clinical Operations Manager (Registered Nurse) to join our leadership team. This position plays a key role in overseeing client care, supervising field staff, and ensuring regulatory compliance in our residential service agency (RSA).

Key Responsibilities Include:

  • Overseeing and coordinating clinical services for home care clients
  • Supervising and supporting nursing assistants and care staff
  • Developing and reviewing individualized care plans
  • Promoting high standards of quality, safety, and documentation compliance
  • Collaborating with the Administrative Director and leadership team
  • Supporting training, auditing, and clinical process improvement initiatives

Ideal Qualifications:

  • Active and unrestricted Maryland RN license (required)
  • Minimum 5 years of nursing experience, including 3 years in home care or similar settings
  • Strong leadership and communication skills
  • Knowledge of COMAR regulations and Medicaid/Medicare home care compliance
  • CPR certification (required)
  • Bachelor’s in Nursing (BSN) required; MSN or Healthcare Administration preferred

Certified Nursing Assistant (CNA)

About the Role:
Liberty Home Care is seeking compassionate, dependable Certified Nursing Assistants (CNA) to provide essential care and support to clients in their homes. This role is ideal for professionals who value dignity, safety, and empathy in client-centered care. CNAs are responsible for assisting with personal care, monitoring health, supporting mobility, and maintaining a safe, clean living environment for clients.

Key Responsibilities Include:

  • Assisting clients with activities of daily living (bathing, grooming, dressing, toileting, feeding)
  • Supporting client mobility, transfers, and repositioning
  • Monitoring and reporting vital signs or changes in condition to the supervising nurse
  • Performing light housekeeping and laundry tasks
  • Preparing and assisting with meals
  • Running errands and accompanying clients to appointments
  • Accurately documenting care and observations
  • Communicating with the care team to ensure consistent, high-quality service

Qualifications:

  • High school diploma or equivalent
  • Active Maryland CNA certification (required)
  • CPR and First Aid certification (preferred)
  • Prior experience in home care or healthcare settings (preferred)
  • Ability to follow care plans and maintain clear, timely documentation
  • Strong communication and interpersonal skills

Additional Requirements:

  • Working knowledge of COMAR (Maryland state home care regulations)
  • Physical ability to lift, transfer, and assist clients with mobility
  • Reliable transportation to client homes
  • Flexibility to work occasional evenings, weekends, or holidays as needed

Home Health Assistant (HHA)

About the Role:
Liberty Home Care is seeking dependable and compassionate Home Care Assistants (HHA) to support our clients in living safely and comfortably in their homes. This position focuses on non-medical assistance, helping individuals with daily activities, household tasks, and mobility support, all under the supervision of a Registered Nurse or Licensed Practical Nurse.

Key Responsibilities Include:

  • Assisting clients with personal care (bathing, dressing, grooming, toileting, feeding)
  • Providing mobility support (transfers, repositioning, walking assistance)
  • Observing and reporting changes in client condition or behavior
  • Performing light housekeeping, laundry, and home organization
  • Preparing meals and assisting with feeding as needed
  • Accompanying clients to appointments or assisting with errands
  • Documenting care provided and maintaining communication with the care team

Ideal Candidate Qualifications:

  • High school diploma or equivalent
  • Certification as a Home Health Aide (HHA) preferred, but not required
  • CPR and First Aid certification preferred
  • Experience in home care or caregiving settings strongly preferred
  • Ability to follow care plans and communicate effectively with supervisors and peers
